Long term treatment of acne

Acne Laser Treatment

Acne is a common, chronic inflammatory condition that can leave permanent scarring and hyperpigmentation.  Main contributing factors are (1) increased sebum production and (2) skin bacteria. Other contributing factors are diet, hormonal variability, injury and mechanical trauma and metabolic disorders.

Acne vulgaris is usually described as mild, moderate or severe. Treatment usually includes topical retinoid, azelaic or salicylic acid, oral antibiotics, benzoyl peroxide and sometime oral contraceptives. Most of these treatments have side effects.  

Blue and red LEDs (Light Emitting Diodes) can be used to treat acne. Blue light kills gram-positive bacteria, and red light reduces inflammation. However, multiple daily or every other day treatments for months may be needed and may only provide mild improvement. Class IV laser such as the Fotona Dynamis are significantly more effective.

For severe acne, more frequent treatments are required. Continuous improvement will be observed over the course of several months.

LASER TREATMENT

Our Nd:YAG laser reduces inflammation and decreases sebaceous glad function, the two major contributing causes to acne vulgaris. This is how it works.

Our Nd:YAG FRAC3 protocol uses very-short, high peak energy pulses. This modality spares the superficial skin layer (it is non-ablative) but creates micro injuries and heat islands in the deeper layers to heat the sebaceous glands to 45-50° Celsius.

This triggers protein changes that reduce sebocyte lipid (fat) synthesis, and triggers a controlled wound-healing signaling shift that turns off the pro inflammatory cytokines (interleukin-1β, tumor necrosis factor-α and interleukin-8), at the same time turning on anti-inflammatory mediators such as TGF-beta and interleukin-10.

We recommend four (4) treatments scheduled every other week.

Pre-Treatment

Preparing the skin for treatment includes:

  • 30 days prior to treatment discontinue use of Acutane.
  • 14 days prior to treatment avoid sun-exposure.
  • Five days prior to treatment discontinue the use of vitamin A products, exfoliating products, lightening creams and alcohol-based products.

Post Treatment

Apply a very thin layer of Clobetasol each night, followed by Alastin Skin Nectar and moisturizer.

During the day, apply medication prescribed by Dr. B or Alastin Regenerative Skin Complex, moisturizer and sunscreen.

Reapply sunscreen three (3) times a day.

Makeup may be used three (3) days after treatment.
